Job Description
Qualified Social Worker, Worcestershire County Council, Adult Hospital Discharge Team - Must be a Qualified Social Worker with a minimum of 2 years+ UK Social Worker experience within an Adult Social Work team
Our client is looking for a qualified Social Worker with 2 years+ UK Social Worker experience within an Adult Social Work team
The team work extremely well together to ensure the safety of vulnerable adults under the close supervision of a supportive Team Manager
The responsibilities will include:
• Working within an integrated onward care team working to the hospital discharge requirements for COVID and looking at strengths based approach to aim for patients returning home where possible
• Involve proportionate assessments to support with an aim for home first
• Review Care Packages
• Respond and assess safeguarding concerns appropriately in line with Care Act
• The service operates 7 days a week, worker needs to be able to work weekends, the 5 working days per week may change and is open to discussion at interview
• 36 hours per week (flexible by negotiation)
• 2 Month Temporary Contract
• Start date subject to DBS and reference clearance

The successful candidate will need to hold a DipSW or equivalent must be registered with the HCPC, have a current DBS and be a car driver.
