Job Description
Qualified Social Worker, Derby City Council, Adult Hospital to Home Team - Must be a Qualified Social Worker with a minimum of 2 years+ UK Social Worker experience within an Adult Social Work team
Our client is looking for a qualified Social Worker with 2 years+ UK Social Worker experience within an Adult Social Work team
The team work extremely well together to ensure the safety of vulnerable adults under the close supervision of a supportive Team Manager
The responsibilities will include:
• Work within the discharge to assess model working with customers within the hospital and the pathways
• Working collaboratively with the Home First and Occupational therapy team and closely with Carelink & Telecare, LAC, Locality teams and Brokerage. Your role will be to complete assessments in all 3 pathways with the aim to returning customers to their homes
• Must be responsive and able to work on your own initiative, have some experience of Assessing and Reviewing customers, with knowledge of equipment and have a re-ablement ethos, you will be required to attend Multi Disciplinary meetings and possess excellent communication, IT and organisational skills
• Post holder will travel across the City of Derby and must have a full UK driving licence, use of car for work purpose together with business use insurance or alternative method of transport
• Liaise with other professionals
• 36 hours per week (flexible by negotiation)
• 2 Month Temporary Contract
• Start date subject to DBS and reference clearance

The successful candidate will need to hold a DipSW or equivalent must be registered with the HCPC, have a current DBS and be a car driver.
