Job Description
Qualified Social Worker, Flintshire Council, Adults Hospital Team - Must be a Qualified Social Worker with a minimum of 2 years+ UK Social Worker experience within an Adult Social Work team
Our client is looking for a qualified Social Worker with 2 years+ UK Social Worker experience within an Adult Social Work team
The team work extremely well together to ensure the safety of vulnerable adults under the close supervision of a supportive Team Manager
The responsibilities will include:
• Cover our winter pressures over the next 4 months, we need someone with experience of working in a hospital settings dealing with older people who are either ready for discharge or are a delayed discharge
• Experience of dealing with Continuing Health Care and DST’s (Decision Support Tool’s) and must be registered with the Care Council for Wales
• Work within a multi-disciplinary team of Social Workers, Occupational Therapists, and Disability Officers working across all three Locality Teams
• Perform a key role taking a complex caseload ensuring the full involvement of service users and carers in the assessment and care planning process
• Liaise with other professionals
• 36 hours per week (flexible by negotiation)
• 4 Month Temporary Contract
• Start date subject to DBS and reference clearance

The successful candidate will need to hold a DipSW or equivalent must be registered with the HCPC, have a current DBS and be a car driver.
