Job Description
Qualified Social Worker, Hull City Council, Adult Learning Disability Team - Must be a Qualified Social Worker with a minimum of 2 years+ UK Social Worker experience within an Adult Social Work team
Our client is looking for a qualified Social Worker with 2 years+ UK Social Worker experience within an Adult Social Work team
The team work extremely well together to ensure the safety of vulnerable adults under the close supervision of a supportive Team Manager
The responsibilities will include:
• Referrals to the teams will come directly from See & Solve Community, for ongoing support following Active Recovery and after a hospital discharge package has been put in place
• The teams work jointly, between Social Work and Occupational Therapy as well as with Sensory and Telecare colleagues, to identify the goals an individual wants to achieve and the best way to achieve these
• Eensure individuals receive timely and proportionate reviews ensuring that an individual’s goals and actions are reflecting changes in their life and support them to be independent
• Undertake proportionate assessments, determine eligibility, and assess risk. Plan care and support and make the best use of available resources to enable people to live the life they choose
• Ensure people are given correct financial advice and information about charging processes of the council
• Home Visits
• Liaise with other professionals
• 36 hours per week (flexible by negotiation)
• 2 Month Temporary Contract
• Start date subject to DBS and reference clearance

The successful candidate will need to hold a DipSW or equivalent must be registered with the HCPC, have a current DBS and be a car driver.
