Job Description
Qualified Social Worker, Lancashire Council, Adults Hospital Team - Must be a Qualified Social Worker with a minimum of 2 years+ UK Social Worker experience within an Adult Social Work team
Our client is looking for a qualified Social Worker with 2 years+ UK Social Worker experience within an Adult Social Work team
The team work extremely well together to ensure the safety of vulnerable adults under the close supervision of a supportive Team Manager
The responsibilities will include:
• Working alongside a well established hospital discharge team with a clear emphasis on supporting and facilitating hospital discharge alongside health colleagues
• During the Covid pandemic and the shift in discharge to assess, there is an increasing workload of supporting the decisions around discharge from hospital for patients going on pathway 3, but also the review process upon discharge from hospital
• As part of this team you will screen and triage referrals, complete social care assessments, support plans and reviews designed to support the delivery of a coordinated response to individual's needs
• The team works in partnership with all internal and external professionals involved in assessment and service provision including Health, police and housing
• You will be an integral part of the team working in partnership with Health and Social Care colleagues across the Fylde & Wyre area, serving the citizens of Lancashire
• Liaise with other professionals
• 36 hours per week(flexible by negotiation)
• 2 Month Temporary Contract
• Start date subject to DBS and reference clearance

The successful candidate will need to hold a DipSW or equivalent must be registered with the HCPC, have a current DBS and be a car driver.
