Job Description

Qualified Social Worker, Waltham Forest Council, Adults Transition Team - Must be a Qualified Social Worker with a minimum of 2 years+ UK Social Worker experience within an Adult Social Work team

Our client is looking for a qualified Social Worker with 2 years+ UK Social Worker experience within an Adult Social Work team

The team work extremely well together to ensure the safety of vulnerable adults under the close supervision of a supportive Team Manager

The responsibilities will include:

• Transition social Worker / Preparing for adulthood working with disabled young people 14 - 25 years old within a 0 - 25 SEND Team. Focusing on young adults 18+
• Key legislations / Knowledge: Care Act 2014, Mental Capacity Act 2005, Mental Capacity Act & Deprivation of Liberty Safeguards 2009, Mental Health Act 1983, Section 42 enquiries, Deprivation of Liberty process, Continuing Health Care, Telecare. It will be an advantage to have knowledge of the Children Act 1989. Children and Families Act 2014, SEND Code of Practice 2014, Care Leavers Act 2000, 2018 , It will be an advantage being a BIA
• Undertaking care act assessments. care and support plan reviews, mental capacity assessments, Community DOL ( Compiling COPDOL11s), attending Education, Health and Care Plan reviews, Looked After Children Reviews in preparation for adulthood for care leavers, Conducting Best Interest Decision Meetings,  Safeguarding Adult (Section 42 enquiries )
Supervision of social care assistants as required
• Good communications skills (Verbal and written), Report writing, Assessment skills, Critical reflection, Analytical skills, Creativity, panel presentation, partnership working with Health, schools, colleges, compiling COPDOL11s for community DOLS, Team player, Duty system
• Liaise with other professionals
• 36 hours per week(flexible by negotiation)
• 2 Month Temporary Contract
• Start date subject to DBS and reference clearance

The successful candidate will need to hold a DipSW or equivalent must be registered with the HCPC, have a current DBS and be a car driver.
