Job Description

Support Worker, Derbyshire County Council, Adult Community Care Team - Must have a minimum of 2 years+ UK experience within an Adult Social Work team

Our client is looking for a support Worker with 2 years+ UK experience within an Adults team

The responsibilities will include:

• Arranging and Supervising of contact
• To assist in the assessment process
• To commission services both internally and externally
• To undertake complete holistic assessments of service user needs on less complex cases, to include a range of outcomes
• Arranging/providing transport for/or acting as escort to clients
• Disseminating information to clients (eg facilities available in the community and/or provided by the Department)
• Assisting Social Workers and others in the implementation of assessment, intervention and care plans
• Liaison with voluntary bodies and other services, agencies or groups working in the area, as appropriate
• Serving on ad hoc bodies/working parties etc.
• Recording of all work undertaken and the preparation of reports/ assessments
• Assisting or running group and community projects in the area, as appropriate
• To undertake such training as is required
• To maintain confidentiality and observe data protection guidelines
• To carry out any other reasonable duties within the overall function, commensurate with the grading and level of responsibility of the post

• Providing services to all client groups if required, including the more routine statutory visiting of children in care
• To be involved, jointly with the Service Manager, in reviewing work and caseload at regular intervals, including the closure and transfer of cases
• To undertake complete holistic assessments of service user needs on less complex cases, to include a range of outcomes including community and residential services, in whatever service user group
• To undertake reviews of service users and their care plans

The successful candidate will need to hold an NVQ3 in Health & Social Care or equivalent, have a current DBS and be a car driver.
